About

Altru Rehabilitation Hospital, part of Altru Health System, is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide high-quality, patient-centered rehabilitation services to individuals with acute, post-surgical, neurologic, orthopedic, and chronic conditions in an inpatient rehab setting. You will complete comprehensive evaluations, develop individualized, evidence-based treatment plans, and deliver interventions designed to improve mobility, strength, balance, functional independence, and overall quality of life. Working collaboratively with physicians, nurses, occupational and speech therapists, case managers, and other members of the interdisciplinary care team, you will help establish functional goals, monitor progress, and adjust plans of care to ensure optimal clinical outcomes. You will educate patients and their families on exercises, safe mobility, use of assistive devices, fall prevention, and strategies for continuing rehabilitation at home or in community settings. Documentation of all evaluations, daily treatments, and discharge summaries will be completed accurately and in a timely manner within the electronic health record, in accordance with regulatory and payer requirements. You will contribute to discharge planning and recommendations for continued therapy services, durable medical equipment, and community resources. The ideal candidate is a licensed Physical Therapist who demonstrates strong clinical reasoning, excellent communication and interpersonal skills, and a commitment to working collaboratively with colleagues to meet the physical, emotional, and educational needs of patients and their families. You should be comfortable managing a diverse caseload, prioritizing care in a fast-paced environment, and adapting treatment approaches to align with each patient’s goals, cultural background, and community support system.
