Role Description The Senior Director of CalAIM Programs will join Zócalo Health during a period of rapid growth and increasing operational complexity. This role exists to ensure consistent execution of ECM, Community Supports (CS), and CHW programs across geographies and health plans. As part of a fast‑growing team, you will be responsible for leading the execution layer of care delivery, ensuring workflows are created, updated, and followed, teams are accountable, and operational issues are surfaced and resolved quickly. This position reports to the Chief Operating Officer.
Execution & Operational Performance
Own day‑to‑day execution of ECM, CS, and CHW programs across all geographies
Ensure visits, engagement, and care delivery activities are completed consistently
Monitor operational KPIs (visit completion, time to first visit, engagement rates)
Identify and resolve breakdowns in execution across teams
Team Leadership & Accountability
Directly manage CHW Team Managers and Senior Manager of Community Supports
Set clear expectations for performance, productivity, and team management
Hold managers accountable for outcomes, coaching, and corrective action
Ensure consistent management practices across geographies
Workflow Adherence & Operational Consistency
Ensure workflows defined by Payor Operations are executed consistently across all teams
Identify where workflows are not being followed and drive correction
Provide structured feedback to Payor Operations on workflow gaps or execution challenges
Partner on refinement of workflows without owning design
Cross‑Functional Prioritization
Make trade‑off decisions across execution, capacity constraints, and workflow requirements
Escalate issues to Payor Operations, CHW Workforce, or Clinical Operations based on root cause
Coordinate with other functions to ensure alignment without overlapping ownership
Issue Resolution & Escalation
Identify system‑level issues impacting care delivery (staffing gaps, workflow gaps, quality issues)
Assign clear ownership based on root cause and track resolution
Escalate unresolved issues to COO or cross‑functional leadership when needed
Qualifications
Direct experience operating CalAIM programs (ECM and/or Community Supports) in California
10–12+ years of experience in healthcare operations with a focus on care delivery or population health
5+ years managing managers or multi‑layered teams in a distributed or field‑based environment
Demonstrated ownership of end‑to‑end program execution, including performance management and operational outcomes
Experience driving consistent execution across multiple geographies or regions
Strong familiarity with Medicaid populations, documentation requirements, and care coordination workflows
Proven ability to manage high variability environments (multiple plans, changing requirements, operational complexity)
Track record of making real‑time operational decisions under competing constraints
Preferred Qualifications
Experience operating within a multi‑plan environment with differing requirements
Experience in value‑based care or risk‑bearing models
Experience scaling operations in a high‑growth or early‑stage organization
