What You'll Do

• Provide hands-on care to patients, ensuring comfort and dignity
• Assist with daily living activities (bathing, dressing, meals, mobility, etc.)
• Monitor and report vital signs or patient changes to nursing staff
• Maintain accurate documentation using EMR/EHR systems
• Work closely with healthcare team members to deliver exceptional, holistic care

What You'll Need

• Active CNA license in the State of Connecticut
• Valid driver's license and reliable transportation
• A caring, dependable, and patient-focused attitude
• Experience in Home Health or Hospice preferred (training available)

• Must have successfully completed the CNA training and competency evaluation program approved by the state.
• CNA License must be active and in good standing.
• Must have a current driver's license and be willing to drive within an assigned territory.
• Must have reliable transportation.
• At least one year of experience and hospice and/or home health experience is strongly preferred.
