A Brief Overview
Ensures safe, accurate and appropriate use of medications, by evaluating prescribers' orders for dosing, allergies and contraindications. Supervises the work of supportive pharmacy personnel. Answers pharmacy-related questions for other health professionals and the general public. Performs pharmacokinetic monitoring and consultations, reports adverse drug events, interventions (iVents), and variances involving medications. Recommends appropriate and cost-effective drug therapy options. Provides medication counseling to patients and their families. Actively participates and facilitates the medication reconciliation process upon admission,
