About
QUALIFICATIONS:
* Preference will be given to a Registered Nurse with a current Alaska State License, or if an IHS employee, a current active RN license from another state. Will consider a BS in Health Science and 3 years' experience working in a hospital setting.
* Three years of experience working with diabetic grant programs/services.
* Previous experience supervising professional staff.
* Knowledge of diabetes and how it effects a person over their life span.
* Certified Diabetic Educator (CDE); or be eligible to obtain national CDE certification within one year of commencing position.
PATIENT CARE:
* Administers patient care based on the philosophy and objectives of the Bristol Bay Area Health Corporation and within the designated scope of practice.
* Participates in planning for appropriate care of patients with gestational diabetes, impaired glucose tolerance and diabetes mellitus.
* Provides the initial evaluation and treatment planning which includes outpatient evaluation, history taking, physical examination and patient education specific to diabetes related care.
* Provides individual, therapeutic planning for patients utilizing clinical observations, and the medical care plan.
* Documents observations on the electronic health record utilizing appropriate format. Records significant normal and all deviations from normal.
* Refers complicated patient problems to the BBAHC physician for consultation or follow- ups of chronic condition.
* Refers patient needing urgent care to the appropriate resource through the BBAHC physician.
* Ensures proper infection control measures are initiated based upon the patient's assessed need.
* Provides culturally appropriate care that is in line with the patient's cultural beliefs. Seeks physician guidance when/if there may be a conflict between what is best for the patient and their acceptance due to cultural norms.
* Schedules clinic appointment times to provide continuity of care for patients.
* Travels to villages providing diabetes patient care.
* Provides thorough and comprehensive patient education based on best practice and ADA guidelines while ensuring that patients understand their care plan.
GRANT AND PROGRAM MANAGEMENT:
* Manages and oversees the Special Diabetes Program for Indians grant services.
* Facilitates the development and communication of effective program policies and procedures.
* Coordinates and reports grant activities with the Grant Management Department aligning efforts with the BBAHC strategy map.
* Manages a team of professional staff in delivering diabetes education and prevention services to 21 health clinics located in rural Alaskan villages.
* Establishes objectives, evaluates staff outcomes, provides annual evaluations.
POSITION TYPE/EXPECTED HOURS OF WORK:This is a full-time position. Days of work are generally Monday through Friday, and hours are generally 8:00 a.m. to 5:00 p.m. Some weekend or evening hours may be necessary hours may go pass 5pm with position requirements. Attendance in accordance with the BBAHC Attendance policy is a job requirement.
