Admissions Coordinator

Crossbridge Hospice is seeking an Admissions Coordinator to facilitate compassionate, timely hospice admissions. In this role, you’ll serve as the first point of contact for patients, families, and referral sources, coordinating assessments, verifying eligibility, and ensuring accurate documentation. You’ll collaborate closely with our interdisciplinary hospice team to support seamless transitions of care, primarily for patients in home and community settings. Ideal candidates bring strong organizational skills, healthcare intake experience, excellent communication, and a deep commitment to dignity, empathy, and patient-centered service.
