Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant to provide in-home health assessments and chronic care management for diverse patient populations. You will perform comprehensive evaluations, review medications, interpret diagnostics, and document findings in EMR to support quality and risk adjustment programs.
