RN Case Manager

Crossbridge Hospice seeks an RN Case Manager to deliver compassionate, home-based end-of-life care. In this role, you will manage a caseload of hospice patients, performing comprehensive assessments, developing individualized care plans, and providing expert pain and symptom management. You will coordinate closely with an interdisciplinary team to address medical, emotional, and spiritual needs, while educating and supporting families through the hospice journey.
