RN Case Manager

Crossbridge Hospice seeks a compassionate RN Case Manager to provide in-home hospice care across the community. In this role, you will manage a caseload of patients, perform comprehensive assessments, create and update care plans, and coordinate services with an interdisciplinary team. You’ll provide expert pain and symptom management, educate and support patients and families, and ensure care reflects our values of dignity, respect, and holistic support. This is a field-based position with meaningful patient relationships in a collaborative, mission-driven environment.
