*Overview*
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care and will be skilled in various nursing practices. As an LPN, you will play a crucial role in supporting patients across different settings, including hospitals, long-term care facilities, and outpatient clinics. Your ability to work collaboratively with healthcare professionals and communicate effectively with patients and their families is essential.
*Responsibilities*
* Provide direct patient care, including administering medications and treatments as prescribed.
* Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
* Assist in the development and implementation of individualized care plans.
* Maintain accurate medical documentation and records in compliance with HIPAA regulations.
* Collaborate with healthcare teams to coordinate patient care and discharge planning.
* Educate patients and families on health management, medication administration, and post-care instructions.
* Perform procedures such as IV insertion, catheterization, phlebotomy, and wound care.
* Support patients with activities of daily living (ADLs) and mobility assistance.
*Skills*
The following skills are essential or beneficial for the role of Licensed Practical Nurse:
* Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
* Experience in critical care, emergency medicine, or acute care settings.
* Knowledge of medical terminology, ICD-9/ICD-10 coding, and medication administration protocols.
* Familiarity with specialized areas such as geriatrics, pediatrics, hospice care, memory care, Alzheimer’s care, and behavioral health.
* Strong patient assessment skills with the ability to recognize changes in patient conditions.
* Experience working with individuals with developmental disabilities or special needs.
* Competence in performing basic life support (BLS) and other emergency interventions.
