Schedule: Day Shift

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 36 per week

Requirements:

• Current PTA License
• BLS Certification
• Driver's License
• Auto Insurance
• Auto Registration
• Annual TB test
• MMR, Varicella, HepB, TDap (10 years), and Annual Flu vaccinations
• Drug Screen
• Physical Examination
• Minimum of 1 year professional experience, preferably in home health

Roles & Responsibilities:

• Provide physical therapy services under the supervision of a licensed physical therapist
• Assist patients with therapeutic exercises and activities to improve mobility and function
• Document patient progress and communicate effectively with the healthcare team
• Follow treatment plans as prescribed by the supervising physical therapist
• Educate patients and families on therapeutic and home exercise programs

Compliance / Notes:

• All requested time off must be disclosed during the submission process
• Orientation and compliance with facility-specific protocols will be required
