Life Care Centers is seeking a dedicated Unit Care Coordinator to support compassionate, patient-first care on one of our healthcare units. In this role, you will serve as a key liaison between residents, families, nursing, therapy, and medical providers to ensure that each patient receives coordinated, high-quality services aligned with their individualized care plan. Working within our senior living and rehabilitation environment, you will help organize and oversee the daily clinical flow of the unit, monitor patient status, and support the interdisciplinary team in delivering safe and timely care.
The Unit Care Coordinator will assist with admissions and discharges, help gather and review clinical information, and support the development, implementation, and evaluation of person-centered care plans. You will coordinate care conferences, communicate changes in condition to appropriate team members, and ensure that documentation is accurate, timely, and compliant with facility, state, and federal standards. This position requires strong organizational skills, attention to detail, and the ability to prioritize in a fast-paced healthcare setting.
Working closely with nursing leadership, you will contribute to quality improvement initiatives, help monitor key clinical indicators, and reinforce best practices in infection control, fall prevention, skin integrity, and overall resident safety. You will also serve as a resource for residents and families, providing clear, compassionate communication, education about care plans and services, and support during transitions of care.
