Salary $95,000-105,000/year
This is a dual role in a small SNF/Rehab setting. Employee will meet and works with individuals to gather information; assesses each individual's needs; assesses the plan and cost of care; and identifies and implements services to meet an individual's needs. Will also be responsible for the evaluation and treatment of clients admitted for treatment.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:
- Evaluation including gathering data from client records, interviews, observation and dialogue with other team members
- Development and implementation of a treatment plan according to physician orders which includes establishing client goals, continuous re-evaluation of progress and appropriate modification of the plan
- Timely preparation of written documentation of client progress including initial evaluations, progress reports, discharge summaries and other requested reports
- Develop a plan of care for each individual to include services such as advocacy/case management, independent living skills, training, peer support, physical therapy, cognitive therapy, etc.
- Increases awareness of client and caregivers about brain injury to better facilitate adjustment and understanding of the process of recovery.
- Assist individuals with related matters including eligibility for benefits through Medicaid, Medicare, Social Security and private insurance.
- Completes and turns in billing information in a timely manner
- Communicates effectively with internal case manager
- Performs other related duties and activities as required.
Minimum Knowledge and Skills required by the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:
Education
- Master’s Degree in Social Work from an accredited school
Certificates, Licenses, and Registrations:
- LCSW preferred, or in progress
Other Requirements:
- Experience in Social work and Case Management preferred
- Travel as needed
Physical Requirements:
- Medium Work.Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
