About
As a Physical Therapist with Trilogy Health Services, you will perform comprehensive evaluations to assess residents’ strength, range of motion, balance, gait, and functional abilities. Using your clinical expertise, you will develop and implement individualized treatment plans focused on realistic, meaningful goals that enhance each resident’s quality of life. Interventions may include therapeutic exercise, balance and gait training, transfer training, functional mobility activities, manual therapy, and the use of modalities as appropriate.
You will collaborate closely with an interdisciplinary team, including nurses, physicians, occupational and speech therapists, and other care professionals, to coordinate care and ensure a unified approach to each resident’s health goals. You will participate in care conferences, communicate progress regularly, and modify treatment plans as residents’ needs change. Education is a key part of this role: you will instruct residents and families on home exercise programs, fall-prevention strategies, safe transfer techniques, and the proper use of assistive devices such as walkers, canes, and wheelchairs.

The ideal candidate is detail-oriented, comfortable working in a fast-paced senior care environment, and able to manage a varied caseload while maintaining high standards of clinical documentation and regulatory compliance. You should bring strong assessment and problem-solving skills, as well as the empathy and patience needed to connect with older adults and their families. Experience in skilled nursing, long-term care, or geriatric rehabilitation is preferred, though we welcome motivated new graduates with relevant clinical rotations.
