About
As a Physical Therapist with Synchrony Rehab, you will collaborate closely with an interdisciplinary team that may include occupational therapists, speech-language pathologists, nurses, physicians, and case managers. You will regularly communicate patient goals, progress, and outcomes, ensuring that each patient receives coordinated, holistic care. You will educate patients and their families on home exercise programs, safe mobility techniques, and strategies for maintaining independence, while promoting long-term wellness and preventing re-injury.
Your daily responsibilities will include conducting detailed assessments, setting measurable functional goals, documenting all services accurately in the Electronic Medical Record, and reassessing and adjusting treatment plans as patients progress. You will uphold all professional, legal, and ethical standards of practice, including state licensure requirements and organizational policies. Time management, clear communication, and strong clinical reasoning are essential as you manage your caseload and prioritize patient needs.
