About
CenterWell Home Health is seeking a compassionate and skilled Speech Language Pathologist to deliver high-quality, in-home therapeutic services to adult and geriatric patients. In this role, you will travel to patients’ homes to evaluate, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders, helping each individual maximize independence and quality of life in the comfort of their own environment. You will complete comprehensive assessments, develop evidence-based, individualized plans of care, and implement therapeutic interventions that align with physician orders and CenterWell’s standards of excellence. Working closely with an interdisciplinary team—including nurses, physical and occupational therapists, and medical social workers—you will coordinate care, communicate patient progress, and adjust treatment plans as needed to ensure safe and effective outcomes. You will educate patients, families, and caregivers on compensatory strategies, home exercise programs, communication techniques, and safety measures, empowering them to actively participate in the care process. Thorough and timely clinical documentation, adherence to regulatory and payer requirements, and strong professional ethics are essential in this position.
