This role plays a critical part in ensuring appropriate reimbursement, quality resident care, and regulatory compliance. The MDS Nurse collaborates with the interdisciplinary team to assess resident needs, develop individualized care plans, and ensure documentation reflects the clinical status of each resident. This position may also provide direct resident care as needed and participate in the facility’s on-call rotation. Coordinate and complete MDS assessments (admission, quarterly, annual, significant change, and discharge) in a timely and accurate manner Ensure compliance with CMS, state, and facility guidelines related to MDS and care planning Initiate, develop, and update individualized care plans based on comprehensive assessments Collaborate with interdisciplinary team members (nursing, therapy, dietary, social services, and providers) to gather and validate resident data Accurately code MDS to reflect residents’ clinical conditions, treatments, and services provided Monitor Case Mix Index (CMI) and support optimization of reimbursement through proper documentation and coding Participate in care plan meetings with residents and families as appropriate Review clinical documentation to ensure it supports MDS coding and regulatory compliance Identify and communicate cha
