*Assisted Living Registered Nurse *
*Duties:*
- Provide comprehensive case management services to residents
- Conduct resident assessments and develop individualized care plans
- Collaborate with healthcare team members to coordinate and facilitate resident care
- Monitor resident progress and adjust care plans as needed
- Utilize utilization management techniques to ensure appropriate use of healthcare resources
-Monitors and follows thru with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications.
- Coordinate admission and discharge planning and ensure smooth transitions of care
- Provide resident education and support to promote optimal health outcomes
- Maintain accurate and up-to-date documentation of patient interactions and care plans
-Communicates changes in residents and delegates tasks to AL Aides
*Skills:*
-Possess a sensitivity and aptitude for working with the elderly.
-Possess leadership/supervisory skills
- Excellent resident assessment skills
- Knowledge of medical terminology and procedures
- Must be able to prioritize and organize work effectively and efficiently.
- Strong communication and interpersonal skills with residents, families and staff
