About
We are seeking a dedicated and compassionate Physical Therapist to join our team. In this role, you will be responsible for evaluating and treating patients with various physical impairments, helping them regain mobility and improve their quality of life. You will work closely with individuals across different age groups, including geriatrics and those requiring orthopedic care, ensuring personalized treatment plans that cater to their specific needs.
*Responsibilities*
* Conduct thorough assessments of patients' physical abilities and limitations.
* Develop and implement individualized care plans based on patient evaluations.
* Provide therapeutic interventions including outpatient therapy, acute care, and home health services.
* Utilize techniques such as trigger point therapy to alleviate pain and improve function.
* Educate patients and their families on treatment plans, exercises, and self-care strategies.
* Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
* Collaborate with other healthcare professionals to ensure comprehensive patient care.
* Maintain accurate documentation of patient evaluations, treatments, and progress notes.
*Qualifications*
* Degree in Physical Therapy from an accredited program.
* Valid state license to practice as a Physical Therapist.
* Experience in geriatrics, orthopedics, or home community care is preferred but not required.
* Strong knowledge of patient care principles and practices in various settings including outpatient and acute care environments.
* Excellent communication skills to effectively educate patients and collaborate with healthcare teams.
* Ability to develop effective care plans tailored to individual patient needs.
* Compassionate demeanor with a strong commitment to improving patients’ quality of life through rehabilitation.

Pay: $40.00 - $45.00 per hour
