About
Trilogy Health Services is seeking a compassionate and skilled Physical Therapist to join our Healthcare & Medical Services team, delivering personalized rehabilitation services to residents in our senior living and long-term care community. In this role, you will work directly with older adults to help them regain function, improve mobility, manage pain, and maintain independence in a setting that values dignity, respect, and person-centered care. As a Physical Therapist with Trilogy Health Services, you will perform comprehensive evaluations to assess residents’ strength, range of motion, balance, gait, and functional abilities. Using your clinical expertise, you will develop and implement individualized treatment plans focused on realistic, meaningful goals that enhance each resident’s quality of life. Interventions may include therapeutic exercise, balance and gait training, transfer training, functional mobility activities, manual therapy, and the use of modalities as appropriate. You will collaborate closely with an interdisciplinary team, including nurses, physicians, occupational and speech therapists, and other care professionals, to coordinate care and ensure a unified approach to each resident’s health goals. You will participate in care conferences, communicate progress regularly, and modify treatment plans as residents’ needs change. Education is a key part of this role: you will instruct residents and families on home exercise programs, fall-prevention strategies, safe transfer techniques, and the proper use of assistive devices such as walkers, canes, and wheelchairs. You should bring strong assessment and problem-solving skills, as well as the empathy and patience needed to connect with older adults and their families. Experience in skilled nursing, long-term care, or geriatric rehabilitation is preferred, though we welcome motivated new graduates with relevant clinical rotations.
