About

Synchrony Rehab is seeking a dedicated Physical Therapist to provide high-quality, patient-centered rehabilitation services within our Healthcare & Medical Services department. In this role, you will work with a diverse caseload of patients across various healthcare settings, delivering individualized care designed to restore function, reduce pain, and enhance overall quality of life. Drawing on your clinical expertise, you will complete comprehensive evaluations, develop evidence-based treatment plans, and implement therapeutic interventions such as therapeutic exercise, gait and balance training, functional mobility training, and manual therapy when appropriate. As a Physical Therapist with Synchrony Rehab, you will collaborate closely with an interdisciplinary team that may include occupational therapists, speech-language pathologists, nurses, physicians, and case managers. You will regularly communicate patient goals, progress, and outcomes, ensuring that each patient receives coordinated, holistic care. You will educate patients and their families on home exercise programs, safe mobility techniques, and strategies for maintaining independence, while promoting long-term wellness and preventing re-injury. Your daily responsibilities will include conducting detailed assessments, setting measurable functional goals, documenting all services accurately in the Electronic Medical Record, and reassessing and adjusting treatment plans as patients progress. You will uphold all professional, legal, and ethical standards of practice, including state licensure requirements and organizational policies. Time management, clear communication, and strong clinical reasoning are essential as you manage your caseload and prioritize patient needs.
