Physical Therapist

Trilogy Health Services is seeking a compassionate, patient-centered Physical Therapist to join our Healthcare & Medical Services team, supporting residents in our senior living and long-term care communities. In this role, you will play a key part in helping older adults maintain and improve their mobility, independence, and overall quality of life through evidence-based physical therapy interventions tailored to individual needs. As a Physical Therapist with Trilogy, you will perform comprehensive evaluations of residents’ physical function, strength, balance, gait, and pain levels. Drawing on your clinical expertise, you will develop and implement individualized treatment plans designed to promote safety, prevent falls, support post-acute recovery, and maximize functional independence in daily activities. Your treatments may include therapeutic exercise, neuromuscular re-education, gait and transfer training, balance programs, and manual therapy when appropriate. You will reassess progress regularly, adjust plans as needed, and clearly communicate changes and recommendations to residents, families, and the interdisciplinary care team. Collaboration is central to this position. You will work closely with nurses, physicians, occupational and speech therapists, and other team members to coordinate comprehensive, resident-focused care. You will participate in care conferences, share your clinical insights, and help develop strategies that support Trilogy’s mission of personalized, compassionate healthcare. Providing ongoing education to residents and caregivers on safe mobility techniques, proper use of assistive devices, and home exercise programs will be a key part of your daily routine. Accurate, timely documentation is essential. You will use our electronic health record system to record evaluations, daily treatment notes, progress updates, and discharge summaries in accordance with professional, regulatory, and company standards. You will also contribute to quality improvement initiatives, fall-prevention efforts, and outcome tracking that help us continuously enhance the care we deliver.
