Occupational Therapist

Avera Health is seeking a compassionate and patient-centered Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based occupational therapy services to patients across the continuum of care, helping individuals restore function, improve independence, and enhance quality of life. Working within Avera Health’s collaborative, mission-driven environment, you will partner closely with physicians, nurses, rehabilitation professionals, and family members to deliver personalized care rooted in respect, hospitality, and stewardship. As an Occupational Therapist at Avera Health, you will evaluate patients with a wide range of physical, cognitive, and psychosocial conditions. You will perform comprehensive assessments, develop individualized treatment plans, and implement interventions focused on activities of daily living, functional mobility, cognitive and perceptual skills, upper extremity function, and adaptive equipment use. You will educate patients and caregivers on home exercise programs, safety strategies, and energy conservation techniques, adapting your approach to each patient’s goals, culture, and home environment. You will document all evaluations, treatment sessions, progress notes, and discharge plans accurately and timely in the electronic medical record, adhering to regulatory, professional, and organizational standards. You will collaborate with the interdisciplinary care team to coordinate services, participate in care conferences, and contribute to discharge planning, ensuring smooth transitions to home, outpatient therapy, or other levels of care.
