About

Avera Health is seeking a compassionate and patient-centered Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based occupational therapy services to patients across the lifespan, helping them maximize independence, safety, and quality of life in collaboration with a multidisciplinary care team. As an Occupational Therapist with Avera Health, you will perform comprehensive evaluations, develop individualized treatment plans, and deliver therapeutic interventions that address functional deficits in areas such as self-care, mobility, cognition, and work or leisure activities. You will provide skilled interventions including therapeutic exercise, neuromuscular re-education, activity modification, splinting, adaptive equipment training, and patient and family education. You will document all care accurately and timely in the electronic medical record, communicate proactively with physicians, nurses, rehabilitation staff, and case management, and participate in care conferences to support coordinated, patient-focused care. You will educate patients and caregivers on home programs, safety strategies, and the use of assistive devices to support safe discharge and community reintegration. You will contribute to fall-prevention, functional mobility, and rehabilitation initiatives within the facility, and you may support services in inpatient, outpatient, or post-acute settings as assigned.
