About
Oak Street Health is seeking a compassionate and mission-driven Primary Care Physician to join our Healthcare & Medical Services team. As a physician in our network of primary care centers for adults on Medicare, you will provide comprehensive, patient-centered care to individuals in underserved communities, with a strong emphasis on prevention, chronic disease management, and improving overall health outcomes. In this role, you will manage a panel of adult and geriatric patients, conducting routine wellness visits, acute care visits, and chronic disease follow-ups. You will take the time to build strong, trusting relationships with patients, understanding their medical, social, and behavioral health needs to develop personalized care plans. By emphasizing preventative healthcare, including screenings, vaccinations, and lifestyle counseling, you will help patients avoid unnecessary hospitalizations and maintain a higher quality of life. You will work in a collaborative, team-based environment alongside nurse practitioners, nurses, medical assistants, care managers, behavioral health specialists, and social workers. Together, you will coordinate care, close care gaps, manage transitions of care, and ensure that each patient receives holistic, integrated support. You will utilize evidence-based guidelines, value-based care principles, and population health tools to monitor and improve patient outcomes. The Primary Care Physician will review and interpret diagnostic tests, order appropriate labs and imaging, prescribe medications, and adjust treatment plans based on patients’ evolving needs and preferences. You will use electronic health records efficiently to document encounters, track quality metrics, and communicate with other members of the care team and external specialists. You will participate in case conferences, quality improvement initiatives, and practice-based learning to continuously enhance the care delivered.
