About
Avera Health is seeking a compassionate, patient-centered Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide high-quality, evidence-based physical therapy services to patients across a variety of settings, helping individuals restore function, improve mobility, reduce pain, and enhance overall quality of life. Working closely with physicians, nurses, and other members of the interdisciplinary care team, you will evaluate patients, develop individualized treatment plans, and deliver therapeutic interventions aligned with Avera Health’s mission of compassion, hospitality, and stewardship. As a Physical Therapist with Avera Health, you will perform comprehensive physical therapy evaluations, including detailed patient histories, physical examinations, and functional assessments. You will interpret clinical findings to establish physical therapy diagnoses and set measurable, patient-centered goals. Using your clinical expertise, you will design and implement individualized treatment plans that may include therapeutic exercise, manual therapy, neuromuscular re-education, gait and balance training, functional mobility interventions, pain management strategies, and patient and family education. You will work in collaboration with referring providers and the broader care team to coordinate patient care across the continuum, communicate clinical findings and progress, and adjust treatment plans based on patient response and changing medical status. You will document all evaluations, treatments, progress notes, and discharge summaries in the electronic medical record in a timely, accurate, and compliant manner, adhering to regulatory, payer, and organizational standards. You will also participate in discharge planning, including recommendations for continued therapy, durable medical equipment, and home exercise programs. The role is typically full-time within a primarily field/on-site or hybrid care environment, depending on the specific clinic or facility assignment, and may include work in outpatient clinics, inpatient units, rehabilitation settings, or other care facilities within the Avera Health network. Work hours may include some flexibility based on patient needs, department scheduling, and coverage requirements.
