Physical Therapist-PRMC

Presbyterian Healthcare Services is seeking a compassionate and skilled Physical Therapist for our PRMC location within our Healthcare & Medical Services department. As a Physical Therapist-PRMC, you will deliver evidence-based, patient-centered care to individuals with a wide range of physical and functional limitations arising from injury, illness, surgery, or chronic conditions. You will perform comprehensive evaluations, develop and implement individualized treatment plans, and use therapeutic exercise, manual therapy, functional training, modalities, and patient education to restore mobility, reduce pain, and optimize independence and quality of life. Working within a collaborative, multidisciplinary team, you will partner closely with physicians, nurses, occupational and speech therapists, case managers, and other healthcare professionals to coordinate safe, effective care and smooth transitions across the continuum, including acute care, inpatient rehabilitation, outpatient, and home-health settings. You will regularly communicate clinical findings, progress, and recommendations to patients, families, and the care team, emphasizing education on self-management, fall prevention, safe mobility techniques, and use of assistive devices. In this role, you will be responsible for timely, accurate documentation in the electronic medical record, adherence to professional and regulatory standards, and active participation in quality and safety initiatives. You will contribute to program development, clinical pathways, and performance improvement projects that advance clinical outcomes, patient satisfaction, and operational efficiency. You will also mentor and support physical therapist assistants, rehab technicians, and students, fostering a learning environment consistent with Presbyterian Healthcare’s values of respect, integrity, accountability, and a commitment to high-quality, safe patient care.
