Speech Language Pathologist

Heritage Health Care Center, managed by HHP Lifeshare, is seeking a skilled and compassionate Speech Language Pathologist to join our Healthcare & Medical Services department. In this role, you will provide comprehensive evaluation and treatment for adults and seniors with speech, language, cognitive-communication, and swallowing disorders in a patient-centered care environment. You will conduct detailed assessments, develop individualized treatment plans, and deliver evidence-based interventions aimed at improving communication abilities, swallowing safety, functional independence, and overall quality of life. Working closely with nurses, physicians, occupational and physical therapists, social workers, and family members, you will participate in interdisciplinary care planning, progress reviews, and discharge planning to ensure coordinated and effective service delivery. Responsibilities include documenting assessments, goals, treatment sessions, and progress notes in accordance with regulatory and facility standards; educating patients, families, and caregivers on communication and swallowing strategies; and contributing to staff training on communication supports and safe feeding techniques. You will adhere to all clinical and ethical standards of practice, maintain compliance with state and federal regulations, and support the organization’s culture of respect, integrity, and healthcare excellence.
