About
Sanford Health is seeking a dedicated RN, Minimum Data Set (MDS), to support our Healthcare & Medical Services team by ensuring accurate and timely completion of resident assessments in our senior living and post-acute care settings. In this role, you will use your clinical expertise and understanding of regulatory guidelines to coordinate the MDS assessment and care planning process for residents, working closely with nursing, therapy, social services, and providers to ensure a holistic and person-centered plan of care. You will be responsible for collecting, validating, and documenting clinical data within the MDS, monitoring changes in resident status, and updating assessments and care plans according to federal, state, and organizational requirements. The RN, MDS, will collaborate with interdisciplinary team members to lead or participate in care conferences, communicate assessment findings, and advocate for residents’ needs and preferences. You will help ensure that documentation supports quality measures, reimbursement accuracy, and compliance with CMS regulations, while promoting high standards of clinical practice and safety. This position requires strong critical thinking and clinical judgment, exceptional attention to detail, and the ability to work both independently and as a collaborative team member in a fast-paced healthcare environment.
