About
In this role, you will provide high-quality outpatient care to a panel of adult patients, conduct thorough evaluations, perform routine and complex primary care visits, and manage acute and chronic conditions such as diabetes, hypertension, heart disease, COPD, and other common medical issues affecting older adults. You will utilize evidence-based guidelines, clinical decision support tools, and population health data to develop individualized care plans that address both medical and social determinants of health.
You will work closely with an interdisciplinary care team, including nurses, medical assistants, care managers, social workers, behavioral health specialists, and pharmacists, to coordinate services, facilitate referrals, and ensure continuity of care across settings. Collaboration, communication, and a strong commitment to equity and inclusion are essential as you partner with colleagues to address barriers to care, reduce hospitalizations, and improve patients’ quality of life.
Oak Street Health emphasizes longer appointment times, smaller patient panels, and proactive outreach, enabling you to truly know your patients and engage them in their care. You will participate in routine follow-up visits, annual wellness visits, transitional care management, and preventive screenings, as well as phone or virtual follow-ups when appropriate. You will review diagnostic tests, lab results, and imaging, adjusting treatment plans as needed, and document all encounters accurately and timely in the electronic health record.
As part of our patient-centered culture, you will contribute to quality improvement initiatives, clinical protocols, and population health projects aimed at closing care gaps and enhancing patient outcomes. You will monitor key performance metrics, such as HEDIS measures and hospital readmission rates, and actively engage in continuous learning and process improvement. You will also participate in case conferences, team huddles, and regular feedback sessions to optimize care delivery.

The ideal candidate is a Primary Care Physician who is passionate about serving diverse, often vulnerable populations, comfortable working in a fast-paced, innovative care model, and committed to building long-term relationships with patients. Strong communication skills, cultural humility, and a team-first mindset are essential. Experience in value-based care, geriatric medicine, or community health is highly valued, though not required.
This is a full-time position in a primarily field/on-site clinical environment at an Oak Street Health center, with possible hybrid use of telehealth where clinically appropriate.
