About

Oak Street Health is seeking a dedicated Primary Care Physician to join our Healthcare & Medical Services team, delivering high-quality, patient-centered care to adults on Medicare in underserved communities. As a Primary Care Physician at Oak Street Health, you will provide comprehensive primary care, focusing on prevention, chronic disease management, and coordinated care to improve patient outcomes and access to essential health services.
In this role, you will manage a panel of adult patients, conducting routine and complex primary care visits, including annual wellness visits, chronic care follow-ups, acute visits, and care coordination. You will perform thorough histories and physical exams, develop and implement evidence-based treatment plans, order and interpret diagnostic tests, manage medications, and provide patient education tailored to each individual’s needs. You will emphasize preventative care, population health, and early intervention to reduce hospitalizations and improve quality of life.
Working within Oak Street Health’s collaborative, team-based care model, you will partner closely with nurse practitioners, physician assistants, nurses, care managers, social workers, behavioral health specialists, and clinical support staff to deliver integrated, holistic care. You will participate in interdisciplinary case conferences, coordinate referrals to specialists and community resources, and contribute to the development of individualized care plans for complex patients, including those with multiple chronic conditions and social determinants of health challenges.
You will leverage Oak Street Health’s technology platforms and electronic health record to document encounters accurately, track outcomes, and use data to drive quality improvement. You will be responsible for meeting or exceeding quality, safety, and patient experience metrics, including value-based care measures, while maintaining a compassionate, respectful, and inclusive approach to every patient interaction.
