About
Working within a multidisciplinary care team that may include physicians, nurses, occupational and speech therapists, social workers, and case managers, you will collaborate closely to coordinate care, communicate treatment progress, and contribute to discharge planning and ongoing care recommendations. You will educate patients, families, and caregivers on home exercise programs, safety strategies, and lifestyle modifications that support long-term health and function. Accurate and timely documentation in the electronic medical record will be an essential part of your daily responsibilities, ensuring regulatory compliance and continuity of care.
