Sanford Health is seeking a dedicated RN, Minimum Data Set (MDS) to support high‑quality resident care and regulatory compliance within our Healthcare & Medical Services team. As an MDS Registered Nurse, you will coordinate and complete comprehensive resident assessments for our senior living and post‑acute care populations, ensuring the accurate and timely completion of the Minimum Data Set in accordance with federal and state regulations. Working closely with interdisciplinary team members—including physicians, nursing staff, therapy, social services, and dietary—you will gather clinical data, validate documentation, and translate complex clinical information into precise MDS entries that drive care planning, reimbursement, and quality metrics. In this role, you will review medical records, medication profiles, treatment plans, and progress notes to capture each resident’s clinical status, functional abilities, and psychosocial needs. You will initiate and update person‑centered care plans based on MDS findings, monitor resident outcomes, and collaborate with the care team to address changes in condition and prevent avoidable complications. Your expertise will help ensure accurate Case Mix Index (CMI) and PDPM/RUG‑related coding, supporting appropriate reimbursement and alignment with Sanford Health’s standards of safe, compassionate care. The RN, MDS serves as a resource and educator for clinical staff on assessment requirements, documentation standards, and regulatory updates related to the MDS process. You will participate in audits and quality improvement initiatives, identify documentation gaps, and provide feedback to enhance accuracy and consistency. You will also assist in survey readiness by maintaining complete, compliant assessment records, responding to surveyor inquiries, and helping implement corrective actions when needed. Strong communication skills, critical thinking, and clinical judgment are essential as you balance detail‑oriented data review with a holistic view of each resident’s needs. This position is ideal for a registered nurse with experience in long‑term care, skilled nursing, or post‑acute rehabilitation who enjoys analytical work, regulatory details, and interdisciplinary collaboration.
