About

RN, Minimum Data Set (MDS) at Sanford Health is a specialized registered nurse role focused on accurately assessing and documenting the clinical, functional, and psychosocial needs of residents in our senior living and long-term care settings. As an MDS RN, you will coordinate and complete comprehensive Minimum Data Set assessments, care area assessments, and related documentation in compliance with all federal and state regulations, payer requirements, and Sanford Health policies. You will collaborate closely with interdisciplinary team members—including physicians, advanced practice providers, therapists, dietitians, social workers, and bedside nursing staff—to ensure care plans are individualized, up to date, and reflective of residents’ goals, preferences, and changing conditions. Drawing on your clinical expertise, you will interpret assessment data, identify risk factors, and help guide care planning to optimize quality outcomes, functional status, and quality of life for each resident. You will monitor key quality indicators, participate in quality improvement initiatives, and support survey readiness by maintaining thorough, timely, and accurate documentation. This role involves reviewing medical records, medication profiles, nursing notes, therapy records, and consultant reports to ensure the MDS is complete and consistent. You will educate and support clinical staff on documentation best practices, RAI/MDS regulations, and care planning standards, serving as a resource on reimbursement, case-mix, and regulatory expectations related to the MDS. This position typically functions in a primarily on-site environment at our senior living or skilled nursing facilities, with potential for some hybrid work dependent on location and operational needs.
