About
As an LPN at Kingston Health Center, you will perform routine nursing procedures such as administering medications, taking vital signs, performing basic wound care, assisting with treatments and therapies, and documenting all care accurately in electronic medical records. You will observe residents for changes in condition, promptly report findings to the RN or provider, and help coordinate appropriate interventions. You will collaborate closely with physicians, therapists, social workers, and other healthcare professionals to ensure that each resident’s physical, emotional, and social needs are met.
You will also provide direct support with activities of daily living as needed, promote resident safety and comfort, and offer compassionate interaction to residents and their families, especially those in rehabilitation and memory care settings. This includes following care plans, using de‑escalation and redirection techniques for residents with cognitive impairments, and maintaining residents’ dignity and independence. Strong communication skills are essential as you will explain care procedures, provide updates to families, and participate in care conferences.
Kingston Health Center fosters a collaborative, compassionate, and patient-centered culture. As part of our team, you will be encouraged to engage in continuous learning, follow best practices in infection control and regulatory compliance, and support quality improvement initiatives. You will adhere to all state and federal long-term care regulations, maintain accurate and timely documentation, and assist with training and mentoring new staff or nursing assistants as appropriate.
This position is ideal for an LPN who is passionate about geriatric care, rehabilitation, and memory care, and who values working in a supportive environment where professional growth is encouraged. You should be comfortable working a rotating schedule that may include some evenings, weekends, and holidays to meet resident needs. In return, Kingston Healthcare offers the opportunity to make a meaningful difference in the lives of residents every day while developing your clinical skills in a team-oriented, resident-focused healthcare setting.
