Physician Assistant

Matrix Medical Network is seeking a dedicated Physician Assistant to join our Healthcare & Medical Services team, delivering high-quality clinical care through in-home health assessments and related medical services. In this role, you will leverage your advanced clinical training to perform comprehensive health histories and physical examinations, review medical records, order and interpret basic diagnostic tests within your scope of practice, and formulate evidence-based care recommendations. Working closely with health plans, government agencies, and an interdisciplinary clinical team, you will help identify gaps in care, support quality improvement initiatives, and contribute to improved outcomes for diverse patient populations. As a Physician Assistant with Matrix Medical Network, you will conduct structured in-home and potential virtual visits, focusing on preventive care, chronic disease management, and risk stratification. You will document findings accurately and efficiently in electronic health records, ensure regulatory and payer-specific compliance, and communicate results and recommendations to collaborating physicians, care managers, and other healthcare professionals. Your work will support health management programs aimed at reducing avoidable hospitalizations and enhancing patients’ understanding of their conditions and treatment plans. You will be expected to practice with a high level of clinical judgment and autonomy while maintaining strong collaborative relationships with supervising and collaborating physicians. Responsibilities include educating patients and families on medications, lifestyle modifications, and follow-up care, identifying social determinants of health that may impact outcomes, and escalating urgent or emergent findings according to established protocols. You will participate in ongoing quality assurance activities, adhere to evidence-based guidelines, and contribute feedback to improve clinical workflows and patient engagement strategies.
