Speech Therapist

CenterWell Home Health is seeking a compassionate and skilled Speech Therapist to provide high-quality, in-home speech-language pathology services to adult and geriatric patients. As part of our Healthcare & Medical Services team, you will travel to patients’ homes to evaluate, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders, helping patients improve their independence, safety, and overall quality of life. Working in close collaboration with nurses, physicians, therapists, caregivers, and family members, you will develop individualized plans of care, set functional goals, and implement evidence-based therapeutic interventions tailored to each patient’s needs. You will perform comprehensive assessments, document findings and progress in the medical record, educate patients and families on home exercises and strategies, and regularly reassess treatment effectiveness, adjusting plans as appropriate.
