Nurse Practitioner- PRN

Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner (PRN) to join our Healthcare & Medical Services team. In this flexible, as-needed role, you will deliver high-quality, patient-centered care primarily through in-home and community-based visits, supporting health plans and government agencies in improving outcomes and reducing overall healthcare costs. As a key clinical provider, you will perform comprehensive health assessments, manage chronic conditions, and provide preventive care and education to a diverse adult population. Leveraging your advanced clinical training, you will obtain detailed medical histories, conduct full physical examinations, order and interpret diagnostic tests as appropriate, and develop individualized care plans that address both acute and chronic health needs. You will document all encounters accurately and efficiently in electronic health records, ensuring compliance with regulatory, quality, and privacy standards. Working both independently in the field and collaboratively with a multidisciplinary team, you will communicate findings and recommendations to primary care providers, care managers, and other stakeholders to support coordinated, value-based care. You will play an integral role in identifying gaps in care, medication adherence issues, social determinants of health, and potential safety risks in the home, escalating concerns and facilitating appropriate follow-up.
