Responsibilities
  • Serve as the primary point of contact for clients, providing compassionate support and guidance throughout their senior living journey.
  • Assess client needs and help customize service plans that align with individual family circumstances and goals.
  • Coordinate communication between families, healthcare providers, and senior living communities to facilitate seamless transitions and ongoing care.
  • Maintain detailed and confidential client records, ensuring accuracy and timely updates.
  • Educate clients and families on available senior care options, resources, and industry best practices.
  • Respond promptly to client inquiries and resolve any concerns with empathy and professionalism.
  • Collaborate with the Platinum Senior Services team to continuously improve client experience and service delivery.
  • Track and report on client progress and outcomes to support organizational goals and client satisfaction.

Requirements

  • Proven experience in client care coordination, social work, healthcare, or a related field.
  • Strong interpersonal and communication skills with the ability to provide compassionate support.
  • Excellent organizational skills and attention to detail for effective case management.
  • Ability to handle sensitive and confidential information with the utmost discretion and professionalism.
  • Familiarity with senior living options and elder care resources preferred.
  • Demonstrated problem-solving skills and the ability to manage multiple cases simultaneously.
  • Empathy, patience, and a passion for helping seniors and their families.
  • Proficiency in using computer software for documentation and communication purposes.
