Physician Assistant

Matrix Medical Network is seeking a dedicated Physician Assistant to join our Healthcare & Medical Services team, providing high-quality, patient-centered care primarily through in-home health assessments and community-based visits. As part of a dynamic organization focused on transforming healthcare delivery, you will use your clinical expertise to perform comprehensive health evaluations, manage acute and chronic conditions within your scope, and contribute to improved outcomes for a diverse patient population. In this role, you will independently conduct thorough patient histories and physical examinations, order and interpret appropriate diagnostic tests, and develop evidence-based care plans in collaboration with supervising physicians and the broader clinical team. You will educate patients and families on disease processes, medications, and lifestyle changes, supporting health promotion and disease prevention in alignment with payor and regulatory requirements. A significant portion of your work will involve in-home assessments for members of health plans and government-sponsored programs, requiring strong clinical judgment, adaptability, and comfort working in varied community settings. You will document all encounters accurately and efficiently in electronic medical records, ensuring compliance with quality and documentation standards. Working closely with nurses, care coordinators, and other healthcare professionals, you will help identify gaps in care, close quality measures, and coordinate follow-up services or referrals as needed. Your input will support risk adjustment, care management, and population health initiatives aimed at reducing healthcare costs while improving patient experience and outcomes.
