Matrix Medical Network is seeking a compassionate and clinically skilled Nurse Practitioner – PRN to join our Healthcare & Medical Services team. In this field-based role, you will perform comprehensive in-home health assessments and focused clinical evaluations for health plan members and other patients served by our clients. Working independently but with the support of a collaborative clinical team, you will leverage your advanced assessment skills to identify medical, functional, and psychosocial needs and help improve patient outcomes while supporting quality and cost-effective care. As a PRN Nurse Practitioner with Matrix Medical Network, you will complete detailed health histories, conduct thorough physical examinations, review medications, and evaluate chronic and acute conditions in the patient’s home or community setting. You will document findings in an electronic medical record system, develop evidence-based recommendations, and communicate relevant information to health plans, primary care providers, and care management teams. You will also provide patient and caregiver education on disease management, medication adherence, preventive care, and healthy lifestyle choices. This role is ideal for a provider who values flexibility and variety in their schedule. As a PRN clinician, you will be able to accept shifts based on business needs and your availability, which may include weekdays, evenings, and occasional weekends. You will need to be comfortable working autonomously in diverse home environments, traveling within an assigned geographic territory, and adapting your approach to meet each patient’s unique needs and circumstances. Core responsibilities include performing comprehensive and condition-specific assessments, ordering and interpreting basic diagnostic tests within scope, identifying high-risk conditions or social determinants that may impact health, and escalating urgent findings following established protocols. You will ensure accurate, timely, and compliant documentation that supports quality reporting, risk adjustment, and care coordination. You will adhere to all clinical guidelines, regulatory standards, and Matrix Medical Network policies while maintaining strict confidentiality and professionalism. To be successful in this position, you should bring strong clinical judgment, excellent communication skills, and the ability to build rapport quickly with patients from diverse backgrounds. Experience with chronic disease management, geriatric or adult primary care, value-based care, or home-based care is highly beneficial. Comfort with technology and EMR systems is essential, as is the ability to manage time effectively and prioritize visits and documentation.
