BreakThrough Physical Therapy is seeking a compassionate and clinically skilled Physical Therapist to join our Healthcare & Medical Services team. As a key member of our patient-centered practice, you will deliver evidence-based physical therapy services that restore function, improve mobility, and reduce pain for a diverse patient population. In this role, you will independently evaluate patients with orthopedic, post-surgical, neurologic, and other musculoskeletal conditions, performing comprehensive examinations that include history taking, movement analysis, range of motion and strength testing, special tests, and functional assessments. You will synthesize your findings to develop individualized, evidence-based plans of care that align with each patient’s goals, medical history, and functional demands. Your daily responsibilities will include delivering hands-on manual therapy, prescribing and progressing therapeutic exercises, providing gait and balance training, and implementing neuromuscular re-education and pain management techniques. You will utilize appropriate therapeutic modalities when indicated, while emphasizing active, functionally oriented interventions that drive long-term results. You will educate patients and their families on diagnosis, prognosis, body mechanics, injury prevention, and home exercise programs to encourage active participation and sustained progress. BreakThrough Physical Therapy operates clinics across multiple states, and you will collaborate closely with a multidisciplinary team of therapists, assistants, physicians, and other healthcare professionals to coordinate care and optimize patient outcomes. You will manage a varied caseload, prioritize treatment sessions effectively, and ensure all care is delivered in alignment with current clinical guidelines and clinic protocols. Accurate, timely documentation in the electronic medical record system, including evaluations, daily notes, progress reports, and discharge summaries, will be a core component of your work. You are comfortable working in a fast-paced outpatient or similar setting, communicating clearly with patients and colleagues, and tracking and reporting functional outcomes.
