Mohawk Valley Health System (MVHS), a leading provider of comprehensive healthcare services in Upstate New York, is seeking a compassionate and skilled Multi Specialty LPN to join our Healthcare & Medical Services team. In this role, you will provide direct, high-quality nursing care to patients across multiple specialties, supporting physicians and Registered Nurses in a dynamic hospital environment. Working within a multidisciplinary team, you will perform patient assessments appropriate to LPN scope, obtain and document vital signs, assist with examinations and procedures, administer medications as ordered, and monitor patients for changes in condition, promptly reporting concerns to the RN or provider. You will coordinate patient care activities across various specialty areas such as medical-surgical, primary care, specialty clinics, and outpatient services, ensuring continuity and a patient-centered approach. Responsibilities include preparing patients and exam rooms, performing treatments such as wound care, catheter care, and specimen collection, maintaining accurate and timely documentation in the electronic medical record, and reinforcing patient and family education regarding diagnoses, medications, and discharge instructions. You will collaborate closely with physicians, RNs, therapists, and ancillary staff, demonstrating strong communication, professionalism, and respect for diversity while upholding infection control, safety, and regulatory standards. The Multi Specialty LPN will also assist with triage, respond to patient calls and concerns, support quality improvement and patient safety initiatives, and help maintain a clean, organized, and efficient clinical environment.
