Life Care Centers is seeking a compassionate and skilled Licensed Physical Therapist Assistant to join our Healthcare & Medical Services team at Life Care Centers of America, a leader in senior living and rehabilitation. In this role, you will provide hands-on support in delivering high-quality physical therapy services to primarily senior residents, working under the direction and supervision of a licensed Physical Therapist. You will help implement individualized treatment plans focused on improving mobility, strength, balance, and functional independence, always keeping patient comfort, dignity, and safety at the center of care. As a key member of a patient-first, family-focused environment, you will conduct therapeutic exercises, assist with gait and balance training, support transfers and mobility, and utilize rehabilitation equipment to help residents achieve their goals. You will closely follow established care plans, monitor patient progress, and provide timely, accurate documentation in the electronic medical record. You will collaborate daily with Physical Therapists, Nurses, Occupational and Speech Therapists, and other care team members to coordinate treatment, communicate changes in condition, and support optimal outcomes. You will educate patients and their families on home exercise programs, safe mobility techniques, equipment use, and strategies to maintain independence. A strong commitment to infection control, fall prevention, and overall patient safety is essential. You will be expected to manage your caseload efficiently, prioritizing treatments, maintaining punctuality with scheduled sessions, and adapting to changing patient needs while upholding compassionate, respectful communication at all times.
