Nurse Practitioner- PRN

Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner – PRN to join our Healthcare & Medical Services team. In this flexible, as-needed role, you will provide high-quality, patient-centered care through in-home health assessments and clinical visits for individuals enrolled in health plans and government-sponsored programs. Working independently in the field while supported by a collaborative clinical team, you will perform comprehensive health evaluations, manage acute and chronic conditions within your scope of practice, and help patients better understand and manage their overall health. As a PRN Nurse Practitioner with Matrix Medical Network, you will conduct head-to-toe physical examinations, review medical histories, reconcile medications, and identify care gaps and risk factors that impact patient outcomes. You will document your findings accurately in electronic health records, develop and communicate evidence-based care recommendations, and coordinate with primary care providers, specialists, and care management teams as needed. You will educate patients and families on disease management, preventive care, and lifestyle modifications, empowering them to take an active role in their health. This role is field-based, requiring regular travel to patients’ homes and community locations in your assigned area, with the possibility of occasional virtual visits depending on patient and organizational needs. You must be comfortable working autonomously, managing a variable PRN schedule, and adapting to diverse home environments and patient populations. Strong clinical judgment, excellent communication skills, and a commitment to regulatory and documentation standards are essential.
