Nurse Practitioner- PRN

Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner – PRN to join our Healthcare & Medical Services team. In this flexible, as-needed role, you will deliver high-quality, patient-centered care through in-home health assessments and related clinical services for health plan members and other populations we serve. As part of a healthcare organization dedicated to improving patient outcomes and reducing costs, you will leverage your clinical expertise to support accurate risk assessment, early identification of health concerns, and effective management of chronic conditions. Working independently in the field, you will travel to patients’ homes to perform comprehensive health evaluations, including detailed medical histories, physical examinations, medication reviews, and screenings aligned with evidence-based guidelines. You will identify care gaps, evaluate functional status and social determinants of health, and provide education that empowers patients to better understand and manage their conditions. Your thorough documentation in electronic health records will support quality initiatives, care coordination, and compliance with payer and regulatory requirements. As a Nurse Practitioner – PRN with Matrix Medical Network, you will collaborate closely with our clinical and quality teams, communicating findings, escalating concerns when needed, and contributing to individualized care plans. You will order or recommend appropriate diagnostic tests within your scope of practice, review and interpret results, and provide clear follow-up recommendations to primary care providers and health plan partners. You will practice according to state practice acts, organizational protocols, and the highest standards of patient safety and infection control. This position is ideal for a Nurse Practitioner who values autonomy, variety in clinical encounters, and meaningful one-on-one time with patients in their home environment. You will work a flexible PRN schedule that may include weekdays, evenings, or weekends based on program needs and your availability.
