Nurse Practitioner or Physician Assistant

Matrix Medical Network is seeking a compassionate and clinically proficient Nurse Practitioner or Physician Assistant to join our Healthcare & Medical Services team. In this role, you will deliver high-quality, patient-centered care through in-home and community-based health assessments, ongoing management of acute and chronic conditions, and coordination of services for health plans and government-sponsored programs. You will leverage your advanced clinical training to perform comprehensive histories and physical examinations, review medical records, order and interpret diagnostic tests, and develop evidence-based care plans that support improved outcomes and reduced healthcare costs. Working directly with patients in their homes and community settings, you will identify gaps in care, address medication adherence and safety, reinforce preventative care and wellness strategies, and provide clear, empathetic education to patients and their families. You will collaborate closely with primary care providers, specialists, care managers, and payers to ensure continuity and quality of care, documenting all encounters accurately and efficiently within electronic health records while adhering to HIPAA and all regulatory, quality, and compliance standards. As a key clinical member of Matrix Medical Network, you will exercise a high degree of autonomy in your day-to-day practice while being supported by a multidisciplinary team of clinicians, quality specialists, and operational staff. You will contribute to quality assurance initiatives, performance improvement projects, and innovative care delivery models designed to transform how healthcare reaches patients where they live.
