Physician Assistant

Matrix Medical Network is seeking a compassionate and detail-oriented Physician Assistant to join our Healthcare & Medical Services team, delivering high-quality clinical care in patients’ homes and community settings. As a key member of a multidisciplinary team, you will perform comprehensive health assessments, manage chronic and acute conditions within your scope of practice, and collaborate closely with physicians, nurses, and care coordinators to improve patient outcomes and reduce avoidable hospitalizations. In this role, you will conduct in-depth medical histories and physical examinations, order and interpret appropriate diagnostic tests, and develop evidence-based care plans that prioritize patient safety, quality, and education. You will review and reconcile medications, identify gaps in care, and provide counseling on disease management, preventive care, and lifestyle modifications. Using electronic health record (EHR) systems and, when appropriate, telehealth technologies, you will document encounters accurately and timely, ensuring compliance with clinical, regulatory, and payer requirements.
