Nurse Practitioner  – Wound Care

Job Summary
We are seeking a Nurse Practitioner or Physician Assistant to provide mobile, evidence-based wound care services. This role focuses on assessing, treating, and managing wounds while ensuring high-quality patient care, compliance, and strong care coordination.

Key Responsibilities

•  Perform comprehensive wound assessments and develop treatment plans 
•  Provide weekly patient visits (or as clinically indicated) 
•  Prescribe treatments and manage wound-related orders within scope 
•  Document all patient care accurately and timely (same-day) 
•  Coordinate care with agencies, physicians, and internal teams 
•  Escalate care when clinically appropriate 

Requirements

•  Active NP or PA license (unrestricted) 
•  Strong clinical judgment and communication skills 
•  Ability to work independently and collaboratively 
•  Willingness to travel locally to patient homes/facilities 

Schedule & Travel

•  Full-Time: Monday–Friday, 8:00 AM – 5:00 PM 
•  Part-Time / Per Diem: Flexible based on patient volume 
•  Travel required (up to ~55 miles) 
•  No on-call required
