Wellness Visit Nurse Practitioner. Nurse Practitioners (NP) needed to conduct In-Home and embedded clinic Health Assessments in San Francisco, CA. and surrounding areas. We offer year-round opportunities: Full-time or Part-Time.

Set your own schedule and earn an excellent income! Our healthcare providers conduct comprehensive, in-home, telemedicine, and mobile Health/Annual Wellness Visits with Medicare patients as part of their health insurance benefits. The evaluations are conducted in a face-to-face manner where the member resides as well as PCP clinic. Within the visit, the clinician reinforces the importance of staying current on covered preventive services while identifying potential gaps in care, which stem from unnecessary hospitalizations and ER visits.

Our Providers enjoy:

• No prescribing or treatment, simply performing consultative assessments/evaluations

Requirement:

• Must be State licensed and Board Certified Nurse Practitioner with NPI.
• Prefer at least 6 months of experience in doing in home wellness exams.
• Previous EMR skills preferred. Bilingual preferred.

The following will be provided:

• Credentialing services and paid training.
• Liability insurance.
• Scheduling of appointments with Medicare members within a certain time frame.
• Web tracking and scheduling tools to maximize your travel time.
• Ipad Tablet/laptop Provided.
• Mileage Reimbursement.
